EZ-CVD

The Easy Cardiovascular Disease (EZ-CVD) Health Risk Assessment is a screening tool that predicts 10-year risk of heart attack and stroke among adults with no history of cardiovascular disease using self-reported information. A history of cardiovascular disease would include a previous heart attack, previous stroke or a doctor telling you that you have coronary heart disease. The score consists of six questions, and the risk of future cardiovascular disease is determined based on the total score points. This tool is designed for people ages 40-79 years old.
